By making an appointment with the spa and agreeing to receive the spa service or treatment, I read, fully understand, and agree the following:

 

I, the undersigned, fully understand and acknowledge that there are inherent risks associated with the spa services and facilities, including but not limited to injuries to my person; injury, loss, or theft of my personal property; illnesses or accidents; and any and all other injuries or complications that may result from the use of these services or facilities, such as slip and falls, allergic reactions, and/or death. I agree to assume all of the risks associated with accepting these services and using these facilities, known or unknown, and hereby state that my participation is completely voluntary.

 

 

 I agree that in providing the services and facilities, the spa does not make any evaluation or recommendation regarding my physical or medical fitness for the requested services. I fully understand my own physical limitations and warrant and represent to the spa that I am in suitable physical condition to accept the spa’s services and use its facilities. It is my sole decision to receive the services or treatment. If I use medications (oral or topical) or have any medical conditions or allergies that may be affected or triggered by any spa products, treatments, or services, I will fully inform the service provider prior to the service so necessary safety measures can be implemented. By taking the spa services, you agree and acknowledge that you do not use medications (oral or topical) or have any medical conditions or allergies, and the spa is not liable for any personal injuries, pain, or discomfort that the services cause me.

 

 

 I release the spa, its parent and subsidiaries, owners, officers, directors, agents, affiliates, employees, and outside contractors, from any and all claims, damages, demands, rights, judgments, costs, and causes of actions that arise from any of the risks mentioned in the preceding paragraphs or any other injury or damage that results from any of the spa’s services or facilities, including any that are due to the negligence of any of the agents named above. I waive any and all such claims, damages, demands, rights (including the right to sue for injury) of any nature, or causes of action and completely release and discharge the spa, its parent and subsidiaries, owners, officers, directors, agents, affiliates, employees, and outside contractors from any and all liability. This release is binding upon my heirs, executors, administrators, and assigns.
